Sometimes you just don't know what's going on with your team. You know something is off, but you can't put the finger on the problem.
Here are some possible reasons a team is fairing poorly:
Confusion. The team lacks a coherent vision. Let team members know what they are supposed to be doing at all times. Discuss the mission statement over and over until everyone is on the same page.
Misalignment. If the team says one thing and does another, you need to discuss values. Define acceptable and unacceptable behavior. Agree to certain accountability standards.
Diffusion. Is each team member doing their own thing? Discuss how the team suffers when members put their own self interest first.
Frustration. Tension can mount when the team lacks resources to do their job properly. If you don't have the budget to do all the things you want, modify your goals to reflect the limitations you and the team are working under.
Doubt. Feedback from you helps everyone on the squad believe that the hard work they put in makes a difference. Make sure every team member, assistants, players, support staff, understands how the team contributes to the organization's overall success.
